A 70-year-old male patient presented with discoloration secondary to internal resorption following trauma, mid-incisal (MID) fractures associated with bruxism, and tooth #9 demonstrating mesial rotation and buccal tipping. Radiographic evaluation revealed calcified canals on teeth #8 and #9, and prior external bleaching attempts failed to resolve the discoloration.
E-max crowns were prepared with intentional modification to correct alignment and improve overall esthetics. Preparations were captured using a digital intraoral laser scanner, and the patient attended a laboratory appointment for custom shade selection. Working alongside the in-house digital lab technician, I utilized 3D CAD software and facial analysis principles learned through continuing education in cosmetic dentistry to design esthetic mock-ups for patient approval. The finalized 3D CAD design was submitted to the laboratory for fabrication. The definitive restorations were returned, verified for fit and esthetics, and successfully cemented into place.
