70-year-old patient presented with chronic generalized periodontitis. Clinical and radiographic evaluation revealed multiple teeth with a poor long-term prognosis. #2-5, 12-15, 18, 19, 30, and 31 exhibited Class II mobility, periapical radiolucencies, secondary caries, and Molars exhibited Class II+ furcation involvement and #4, 5, 12, and 13. Based on periodontal status and remaining tooth structure, the overall prognosis of the dentition was considered poor with an estimated five-year prognosis approaching hopeless.
Full-mouth extraction with fabrication of complete dentures (CD/CD) was discussed as a definitive treatment option. After reviewing the findings and treatment options, the patient elected to have only the hopeless teeth extracted and proceed with fabrication of transitional removable partial dentures (RPD/RPD).
The patient was informed that the long-term prognosis for the remaining dentition was unfavorable and that progression of periodontal disease would likely require future extractions and eventual fabrication of complete dentures. The importance of regular follow-up visits and meticulous oral hygiene was emphasized.
Transitional interim RPDs were fabricated to restore function and esthetics while allowing flexibility in treatment. The acrylic design allows additional teeth to be added to the prosthesis as extractions become necessary with progression of the disease.
